The Discharge Planner is responsible for assisting patients and families with discharge planning by coordinating and implementing post-hospital care. This involves evaluating and assessing patient needs, financial status, insurance coverage, and available community resources in collaboration with physicians, nursing staff, and case managers.
Minimum requirements include being a high school graduate with the ability to communicate verbally, read, write, and speak English fluently, and possess basic computer skills and knowledge of community resources. Preferred qualifications include an Associate's degree and three to five years of experience in healthcare or a Social Work-related field.

The Discharge Planner assists patients and families with discharge planning. The Discharge Planner coordinates and implements post hospital care by evaluating and assessing the patients’ needs, financial status/insurance coverage and community resources available. Discharge planning is coordinated with physicians, Nursing, Case Managers/Care Coordinators and the patients along with their families/significant others who will be involved in their post-hospital care.
