The Care Coordinator II proactively assesses, plans, implements, monitors, and evaluates services to create an individualized care plan for the patient across the continuum of care, addressing multiple environments and healthcare system needs. This role focuses on utilizing strong customer service skills and knowledge of goal setting to ensure optimal and cost-effective patient outcomes while adhering to MCO contract procedures.
Candidates must meet specific education and experience thresholds, such as an Associate’s degree with five years of healthcare experience, or a Bachelor’s degree with four years, or a Master’s degree with two years. Required qualifications include computer proficiency, strong communication skills, comfort with change, and obtaining First Aid/BLS/CPR certifications within 30 days of hire.

Under licensed supervision, the Care Coordinator II – Delegated works closely with the patient/family and the Interdisciplinary Care Team to proactively assess, plan, implement, monitor and evaluate options and services to effect an appropriate, individualized plan for the patient across the continuum of care. The process addresses the patient’s multiple environments of care and healthcare system needs. The position requires a focus on customer service skills, knowledge of goal setting, and measuring outcomes to effectively ensure the patient has the most optimal outcomes provided in the most cost-effective manner. Adheres to procedures and timelines as specified in the Managed Care Organization (MCO) contract and PMS procedures.
