The Wraparound Specialist will provide navigation assistance and referrals to patients to improve their health by addressing social barriers, focusing on transportation, food, housing, and education/employment needs. Responsibilities include conducting in-depth patient assessments using the PRAPARE tool, developing action plans, providing referrals, coaching, and ensuring follow-up on progress.
Candidates must possess a High School Diploma or GED, with an Associate's Degree preferred, and must be eligible for or hold Community Health Worker (CHW) certification in Texas. A minimum of three years of experience in health navigation or patient assistance is required, along with fluency in English and proficiency in a second language such as Spanish, Arabic, or Chinese.

The Wraparound Specialist will provide navigation assistance and referrals to patients of Asian American Health Coalition (AAHC) DBA HOPE Clinic to improve their health by addressing social barriers, collectively called social determinants of health. Responsible for coordination of activities based on identified social barriers to optimal health, including those revolving around transportation, food, housing and education/employment needs. Provides education on and acts as a bridge to the US health system for immigrants and refugees.
Conducts in-depth assessment of patients identified as having social issues through the PRAPARE tool to identify strengths and opportunities for growth; In conjunction with patient, develop a plan to address social issues; Provides referrals, coaching and mentoring to ensure patients have opportunities to address social needs; Follow-up with patients to determine progress and provide coaching/mentoring as required. Stay up to date with social service resources; Service and respond to customer and patient questions, issues and/or concerns; Securely and accurately document all patient information; Interpret dictation and respond accordingly; Submit Agency forms and documentation in a timely manner. (Improvement activities, response to memorandums, travel reconciliation, supply requests, leave requests, monthly in-service); Ensure timely submission of paperwork and reports; Communicate with other clinics, specialists, physicians, management, and billing; Maintain updated patient information and referral forms; Attend on-site/off-site community engagement activities and on-site/off-site clinic events as needed; Positively contribute to staff morale and corporate culture; Ensure that work assignments and information gathered from day-to-day work is not shared with anyone and protects all patient and agency information. Is knowledgeable of and maintains HIPAA standards of privacy and confidentiality; Ensure that our patients get the best possible care; Schedule and contact patients for required programs and appointments as assigned; Actively participate in agency performance improvement initiatives such as chronic care management and utilizes the approved methodology such as PDSA; Perform other duties as assigned to support HOPE Clinic’s Mission, Vision and Values. Attend on-site/off-site community engagement activities, clinic events, and/or training as needed; Perform other duties as assigned to support HOPE Clinic’s Mission, Vision, and Values.
QUALIFICATION REQUIREMENTS: Community Health Worker certified by State of Texas or eligible to apply for certification. Minimum of three year experience in health navigation, and/or clinic patient assistance Knowledge of and experience in developing, implementing and working in a community outreach program Knowledge of existing social service programs available in Houston area and Harris County Office equipment (e.g., computer, copier) Microsoft Office Must be able to utilize the Internet Strong written and oral communication skills Strong customer service skills Must be detailed oriented and able to handle multi-tasks Detail oriented Must be able to handle multiple tasks, complexity, and diversity of customers Must be fully vaccinated against COVID-19.
High School diploma or GED is required; Associates Degree is preferred. Experience in the medical field and referral/patient navigation. Texas certified Community Health Worker (CHW) preferred (or willing to obtain this certification).
Bilingual (Vietnamese, Chinese, Arabic, and/or Spanish with English) is required. Above average skills in language ability as well as public speaking and writing. Must have good transportation and a valid Texas Driver’s license.
