Rehabilitation Nurses facilitate direct patient care and skilled nursing visits, performing tasks such as medication management, wound care, vital sign monitoring, and patient education according to the established plan of care. Duties include completing patient care tasks, following and reevaluating the plan of care, working scheduled shifts, and completing all required clinical nurse notes and skilled care services.
Candidates must hold a current Licensed Practical Nurse or Registered Nurse license from the Virginia Board of Nursing and must possess experience working in a home care agency with direct patient care experience. The role requires dedicated and dependable individuals to facilitate patient care.
