The coordinator will assess and evaluate individual needs, developing strength-based, stage-wise treatment plans matched to person-centered goals. Responsibilities also include completing necessary documentation and collaborating with community partners to secure services.
Candidates must be a CMHP according to Medicaid guidelines, possessing either a Bachelor's degree with three years of supervised experience or a Master's prepared LLMSW/LMSW in Michigan with specialized training and one year of relevant experience. A valid motor vehicle operator's license is also mandatory.
