The role is responsible for assessing and monitoring safe discharge plans by promoting quality, cost-effective options for acute care patients as they transition through the continuum of care. This involves participating in interdisciplinary rounds, ensuring medical appropriateness criteria are met, and reevaluating the plan of care based on changing patient needs.
Candidates must possess a current California RN License and American Heart Association recognized BLS - Healthcare Provider certification, with a minimum of 1 to 3 years of directly related RN experience required. Preferred qualifications include a Bachelor's Degree and/or National Certification in Utilization Review or Case Management, along with 3 to 5 years of acute care nursing experience.
