Case Managers apply systems and information to improve healthcare practice, managing medical, social, and mental health conditions collaboratively to achieve optimal patient wellness and coordinated, cost-effective care. Responsibilities include assessing patient needs, applying InterQual criteria for service determination, and developing/coordinating transition and discharge plans with physicians and the interdisciplinary team.
Candidates must have a minimum of 3 years of acute care healthcare experience and be a graduate of an accredited School of Nursing, holding a valid Registered Nurse (RN) license for the State of MS. Preferred qualifications include a Bachelor's Degree in Nursing or Allied Health, and certification by the Case Management Society of America.
Case Managers will apply systems, science, incentives, and information to improve healthcare practice and assist patients and their support systems to become engaged in a collaborative process designed to manage medical/social/mental health conditions more effectively. The case manager's objective is to achieve an optimal level of wellness for patients and improve coordination of care while providing cost effective, non-duplicative services. Performs all other duties as assigned.
Registered Nurse (RN) by the State Board of Nursing Required.
