The Nurse Practitioner supports hospice and palliative patients and caregivers by providing clinical expertise to the Interdisciplinary Team while traveling throughout the Pioneer Valley to conduct patient assessments and palliative consultations. Essential functions include performing face-to-face encounters, evaluating patients in various settings, and collaborating on appropriate care plans while maintaining dignity and comfort.
Candidates must possess a Registered Nurse license and an MS in nursing, along with current Massachusetts licensure as a certified Adult or Family Nurse Practitioner and CPR certification. A minimum of three years of healthcare experience is required, with preference given to experience in hospice, homecare, or Long Term Care.

The Advanced Practice Provider is a highly skilled professional with the primary responsibility of providing palliative care consultations and face-to-face hospice assessments (as appropriate) to patients and their families within the scope of practice of a Nurse Practitioner or Physician Associate in the state of Massachusetts.
• Master’s degree or higher in nursing or a physician associate program
1. Experience working in an interdisciplinary team is highly desired. 2. Excellent organizational and time management skills. 3. Ability to relate professionally with a range of individuals and situations. 4. Knowledge and understanding of palliative and hospice philosophy and approach to care.
1. Communicate effectively in writing, on the telephone, and in person. 2. Sit or stand continuously for up to two hours. 3. Lift and/or carry items weighing up to 20lbs. 4. Complete written reports and forms 5. Drive vehicle to work appointments. 6. Operate office equipment (fax, copier, word processor, pager, etc.)
