The Hospice Medical Social Worker provides in-home services by evaluating patients' psychosocial needs, developing personalized care plans, and assessing the impact of terminal illness on patients and families. Responsibilities also include counseling, crisis intervention, educating caregivers on advanced directives, and assisting with future planning needs.
Candidates must possess a Master's or Doctoral degree from a Council on Social Work Education accredited school and hold the required Social Worker licensure for the state of practice. Additional requirements include a valid driver's license, reliable transportation, proof of automobile insurance, current CPR certification, and the ability to pass background and sex offender checks.
