The RN Care Manager will conduct home visits to assess safety, gather health status changes, and provide clinical coaching, including medication review. Ongoing support and coaching to caregivers will be provided via an app, along with resource referrals and documentation of records.
Candidates must possess an active Registered Nurse (RN) License in the state of employment and have a minimum of two years of recent experience in direct care for elders or adults with disabilities. A valid driver’s license, automobile insurance, and the ability to travel within the assigned geographic area are also mandatory.

Develop trusting relationships with caregivers to influence change and reduce caregiver burnout Document and review records for accuracy and timeliness, ensuring compliance with regulatory and accreditation standards Other projects and duties as assigned. Perform other duties as assigned.
Must hold an active Registered Nurse (RN) License in the state of employment Minimum two years of recent experience in direct care of elders or adults with disabilities Ability to travel (usually by automobile) within assigned geographic area; valid driver’s license and automobile insurance is required Experience in case management, care planning and assessment for cases including but not limited to eldercare, people with disabilities, and other complex medical needs in a health care setting Proficiency in common software and web-based applications (e.g. MS Office, case management systems) Additional state-specific qualifications may be required.

The actual wage offered may be lower or higher depending on budget and candidate experience, knowledge, skills, qualifications, and geographic location. #LI-Hybrid (Role is hybrid; candidates must reside in the state of GA.)
