The Physical Therapist will provide comprehensive skilled physical therapy services in patients’ homes as ordered by the attending physician, monitoring and reporting patient progress. Key duties include developing individualized care plans, educating patients and families, and ensuring continuity of care across the recovery journey.
Candidates must possess a current Physical Therapist license in the state of employment and a minimum of one year of acute care experience is required, with home care experience being preferred. A valid driver’s license and auto insurance are mandatory for travel to patient locations.
