• Our benefits package includes: Medical, dental, and vision insurance with flexible options Paid time off 401(k) participation opportunities Company-paid life insurance Employee Assistance Program (EAP) HOW YOU'LL WORK Provide comprehensive skilled services as ordered by the attending physician Educate patients and family members while ensuring patient safety Maintain accurate documentation and comply with all legal and regulatory standards QUALIFICATIONS & ATTRIBUTES Current Physical Therapist license in the state of employment Minimum of one year of acute care experience required Home care experience preferred Strong communication and interpersonal skills Knowledge of therapy best practices Sound clinical judgment and problem-solving ability Detail-oriented and observant Strong time management and prioritization skills Valid driver’s license and auto insurance (listed driver) MAJOR AREAS OF RESPONSIBILITY Provide skilled physical therapy in patients’ homes as prescribed Monitor, record, and report patient progress and changes Perform OASIS assessments and adjust care plans as needed Complete initial evaluations within 48–72 hours of referral Develop and implement individualized care plans Help reduce re-hospitalizations through proactive care Deliver therapeutic treatments (exercise, massage, heat, etc.) Coordinate outpatient services when necessary Ensure continuity of care (admissions, transfers, discharges) Educate patients and families on conditions and care plans Train patients in the use of assistive equipment Ensure awareness of financial responsibilities for equipment Supervise PTAs and other support staff as appropriate Evaluate and adjust plans of care based on patient response Communicate with care team members regarding patient status Complete documentation per agency and Medicare guidelines Participate in audits, meetings, and training sessions Maintain licensure and professional standards ESSENTIAL FUNCTIONS Travel to patient locations as scheduled Maintain daily attendance at assigned visits
