The Case Management Assistant will be responsible for coordinating and facilitating Team Conference meetings, gathering and maintaining essential Case Management documentation like referrals and admissions, and coordinating daily assignment updates. This role also involves completing pre- and post-discharge activities to ensure preparedness for patients, families, and clinicians.
A high school diploma or equivalent is preferred, and proficiency in Microsoft Office products along with strong typing skills are required for this role. Preferred qualifications include knowledge of medical terminology and experience with insurance/payor systems.
