The Social Worker will provide essential social work services across various settings including acute care, maternal child care, emergency department, home health, and hospice for patients with acute, chronic, or terminal illnesses and their families. Key duties involve identifying psychosocial needs, developing strength-based care plans, managing discharge planning, providing crisis intervention, and collaborating with multidisciplinary teams and community resources.
Candidates must possess a minimum of a Bachelor's degree in Social Work, with preferred experience of one to two years in a medical field. Preferred licensure includes Licensed Master Social Work or Licensed Specialist Clinical Social Worker, although a Licensed Bachelor of Social Work is the minimum requirement.
Provide social work services to patients in acute care, maternal child care, emergency department, home health/ hospice setting for patients with acute, chronic, or terminal illness and their families. Services to patient and families include: identification of psychosocial needs, develops, and implements plan of care from the strengths perspective responsive to patient/family concerns, health care status, available resources, age specific problems and clinical impressions; discharge planning, crisis intervention, information and referral, protective services, patient family education, collaboration with the multidisciplinary care team, and building connections with community resources.
