The contracted Medical Nutrition Therapist will provide medically necessary nutritional services to children and youth in foster care programs and through Social Care Networks, focusing on preventing nutrition-related diseases and promoting health outcomes. Key duties include creating treatment plans, conducting initial assessments and counseling sessions, monitoring progress, and communicating with various internal and external stakeholders.
Candidates must be fluent in Spanish and hold RD (Registered Dietitian) or CDN (Certified Dietitian Nutritionist) credentials, with a Master's Degree preferred. Required experience involves working with individuals with significant medical and mental health backgrounds, understanding trauma, and serving diverse youth populations.

The contracted Medical Nutrition Therapist is responsible for providing nutritional services to children and youth within the Enhanced Family Foster Care and Therapeutic Foster Boarding Home Program, as well as provide Enhanced Nutrition Services through Social Care Networks. Services must be medically necessary and recommended by a physician or other licensed practitioner of the healing arts acting within his or her scope of practice under state law.
