Primary Care Nurse Practitioners will provide routine and urgent assessments, manage chronic diseases, and conduct post-discharge follow-ups. They will also coordinate with facility staff and ensure compliance with CMS regulations.
Candidates must have active CA NP and RN licenses, along with a furnishing license, NPI, DEA, and BLS certification. Geriatrics or facility-based experience is preferred, and proficiency in EMR is required.
