The Outreach Worker provides direct case management and support to individuals to help them develop daily living skills and achieve personal goals. Responsibilities include implementing service plans, providing crisis intervention, maintaining records, and assisting with community integration.
Candidates must have at least one year of related experience and a high school diploma or GED, with a bachelor's degree preferred. Applicants are required to have a valid driver's license, access to a reliable vehicle, and the ability to obtain CPR, First Aid, and MAP certifications.
Overview As an Outreach Worker, you will provide high quality, direct case management services to the individuals Vinfen supports. Your support and instruction will help in developing and enhancing activities of daily living, achieve goals, overcome barriers, and become more independent. In your role you will work together with the people we serve, their families, friends, employers, and other support staff to transform their lives.
