The Case Manager is responsible for the full client care cycle, including assessing, planning, and implementing services to meet the health and human needs of veterans and their families. They will maintain caseloads, document clinical activities, and facilitate care coordination to help clients achieve wellness and autonomy.
Candidates must possess an Associate’s degree in a behavioral science or a related field, or equivalent work experience. A valid driver's license and auto insurance are required, along with proficiency in Microsoft applications and case management principles.
