Role Overview We're seeking experienced Primary Care Nurse Practitioners to provide on-site care to residents in Assisted Living and RCFE communities across the Inland Empire.

What You'll Do Conduct routine and urgent assessments; manage chronic disease across a dedicated patient panel Perform post-discharge follow-ups, medication reconciliation, and coordinate labs and diagnostics Collaborate with facility staff, specialists, families, and POAs to ensure seamless continuity of care Complete same-day documentation in compliance with CMS and regulatory standards Serve as the primary on-site clinical provider, building lasting relationships with residents and care teams Schedule & Setting Part-time position with a flexible weekday schedule.
