The Nurse Case Manager assesses and plans patient discharge needs in collaboration with an interdisciplinary team to ensure a smooth transition to the next level of care. They also monitor inpatient length of stay, identify barriers to discharge, and manage utilization processes.
Candidates must be a graduate of an accredited school of nursing and hold a valid Registered Nurse (RN) license in Missouri. A minimum of three years of acute care experience is required, with familiarity with EMR systems.
