The Nurse Practitioner provides comprehensive hospice care to terminally ill patients in their homes and facilities, including assessments, symptom management, and patient/family education. They also collaborate with the interdisciplinary group to develop and implement care plans while maintaining accurate clinical records.
Candidates must be a licensed Registered Nurse and Nurse Practitioner with a graduate degree from an accredited nursing school. A minimum of 2 years of experience as a registered nurse is preferred, along with a valid driver's license and current CPR certification.
