The LPN will provide clinical oversight, medication management, and direct care to elders in a memory care setting. They will also act as a clinical lead, coordinating care plans and fostering positive communication among the care team.
Candidates must hold a valid NYS Nursing Licensure (LPN). While one year of elder or direct care experience is preferred, it is not strictly required.

LPN $25.00 - $29.00 (+$2.00/hour evening shift differential, +1.00/hour night shift differential, +$3.00/hour incentive on weekends)
Day, Evening, night - All candidates must be willing to work rotating weekends and rotating holidays
1 year of elder care or direct care experience is preferred but not required. NYS Nursing Licensure (LPN) required.
$500 retention bonus available!
RPH is looking for LPN candidates who are passionate about caring for elders. Responsibilities include but are not limited to; Medication monitoring and administration, acting as clinical lead to staff, case management, care planning and oversight, personal care assistance as needed, meal planning and enrichment recreation as needed.
