The Care Coordinator will work with medical staff to implement chronic disease management programs and coordinate patient care visits. They will also serve as a communication link between patients and staff while maintaining strict HIPAA confidentiality.
Candidates must have a minimum of three years of clinical experience and a valid Florida PN or RN license. Proficiency in medical records review and disease management is preferred.
