Provide targeted case management, including treatment planning, counseling, and direct skill teaching to support clients in maintaining housing and benefits. Coordinate care by networking with service providers, landlords, and natural supports while maintaining accurate documentation.
Requires a Bachelor’s degree in human services or equivalent experience and at least two years of experience working with dually diagnosed individuals. A valid driver's license is mandatory, and CPR/First-Aid certification is desired within six months of hire.

About the Position: Supportive Housing Specialist
Education: Bachelor’s Degree in human services or equivalent education and

$19.28/hr minimum, and up commensurate upon experience.

Many of our positions offer program/location differentials and/or sign-on incentives. (Please note, for part-time positions this range would be pro-rated based on hours worked)
