Job description
Purpose and scope
Supports the delivery of excellent nutritional care to patients by working under the supervision of the facility registered dietitian (RD) in an assigned area. Performs hands-on duties with patients as assigned.
Principal
DUTIES
AND
RESPONSIBILITIES: § Initiate standard procedures, such as completing and/or following up on nutrition screening for assigned units/patients § Perform routine activities based on diet order and/or policies and procedures § Complete the intake process for a new facility patient and report to the facility RD when a patient’s data suggests the need for evaluation § Actively participates in nutrition care by contributing information and observations, monitoring nutrition supplement compliance § Provide education on prescribed diets §
Assist the
RD with the collection of data and other activities: o Performance of nutrition assessments o Development of nutrition-related priorities, goals and objectives o Implementation of nutrition care plans § Assist with providing ongoing management and revision of nutrition interventions reflecting patient response to nutrition care including timely reporting of observations and communications with patient, family or caregiver § Utilize the application of the Nutrition Care Process (NCP) § Collect patient information, including assessment and evaluation, reporting observations, and communicating with patients, family or caregivers § Assist in distributing patients’ monthly laboratory information § Provide nutritional education materials deemed appropriate for patients by the RD § Collaborate with the RD and healthcare providers, including nephrologists, clinical managers/facility assistants, social workers, nurses, patient care technicians to ensure cohesive nutritional education § Assist with scheduling and conducting telehealth between remote registered dietitians and patients § Maintain accurate patient records with up-to-date contact information § Assist with gathering data to complete assessments § Complete patient care assessments as directed by the registered dietitian
