The Nurse Practitioner will conduct comprehensive health assessments, including reviewing medical history and medication adherence, and collaborate with various healthcare professionals. They will provide services at the member's residence, ensuring personalized care and improved health outcomes.
Candidates must have a Master's Degree or equivalent experience, current NP licensure, and board certification in relevant specialties. A minimum of one year of NP experience is preferred, but new graduates are encouraged to apply.
During a visit that can last up to one hour, Matrix providers review and observe a member’s current health, medical history, medication adherence, social environment and other risks. This provides unmatched insight into a member’s overall health and well-being that can be difficult to capture during routine office visits. The Matrix Comprehensive Health Assessment (CHA) helps to improve quality of care and allows us to potentially close multiple care gaps with a single visit. Our providers: Conduct adult / geriatric assessments to include medical history, diagnosis and treatment, health education, physician referrals, case management referrals, follow-up and clear documentation according to Matrix guidelines and protocols Administer and order point of care and lab-based testing as needed Work with physicians, case managers, social workers, family members, key caregivers, and ancillary medical personnel as appropriate Collaborate with Primary Care Physician (PCP) on patient education and follow up as necessary Provide services at the member’s place of residence which can be in their homes or Skilled Nursing Facilities Benefits of Working at Matrix Consistency – Although we conduct assessments Sunday – Saturday from 8 am – 7 pm, our full-time (FT) providers typically work Monday – Friday from 8:00 to 5:30. Competitive Pay – Earn an annual base salary ranging from $95,500 - $158,400 based on market compensation analysis and experience. Incentive Program – Eligible for FT Clinician Incentive Plan in support of leaving no patient behind which includes monthly (uncapped) and year end (up to $25K) incentive opportunities. In 2025, the average payout from this plan was $1,500 per month and $7,500 at year end, totaling $25,500 annually.
