The Physical Therapist evaluates patient mobility, strength, and functional levels to establish personalized care goals. They provide home-based therapy, recommend safety equipment, and educate patients on proper techniques to improve their overall health and activity.
Candidates must hold a current and active Physical Therapy license in the State of California and a BLS certification. A minimum of one year of experience as a licensed Physical Therapist is required, with home health experience considered a plus.
Our Licensed Physical Therapists (PT) work in collaboration with the doctor, patient and the patient support system to provide best in class care for return to health and activity. As a Physical Therapist, you are responsible for the evaluation of mobility, strength and level of function. With this information you will establish goals to improve balance, walking, strength, as well as patient education. In the comfort of the patients’ home, the Physical Therapist assesses the home environment to ensure safe mobility is achieved. With your knowledge of best in class techniques, you will recommend equipment to enhance therapy, as well educate the patient on proper use. In the unique setting of Home Health, each Physical Therapist sets their own schedule. You will find a great level of independence with the established support of an industry leader.
Minimum one (1) year experience working as a Licensed Physical Therapist Current/active Physical Therapy License – State of California BLS Certification General knowledge and understanding of clinical verbiage Strong attention to detail for use in chart documentation Excellent time management/calendaring skills and ability to deliver documentation timely Strong experience using computers and computer systems Experience using Microsoft Suite (Word, Excel, Outlook), Gmail Suite or other systems Excellent written and verbal communication Desired: One (1) year Home Health a plus Advanced computer system knowledge Bilingual (English/Spanish, English/Tagalog, English/Armenian) a plus *Compensation starting rate indicated on this posting is based on Employees working a Full-Time schedule.
