Visiting Home Care PCA/CNA Every Monday Wednesday Friday 8a-230p

• Job Summary We are looking for a Home Health Aide to join our team!
• You will directly work with patients, following a one-on-one care plan in patient homes with visiting 2 clients per day.
• You operate with a big heart, not only providing personalized and attentive care but by building genuine relationships with those you serve.
• You are detail-oriented, highly organized, and committed to creating a healthy environment that meets patient needs.
• Strong verbal and written communication skills are needed to succeed in this role.
• Prior Home Health or Hospice experience is a plus.

Responsibilities

• Position is every Monday Wednesday and Friday from 8a-2p in the DeKalb County area.
• Must be comfortable working with 2 clients per day
• Assist patients with daily activities such as moving in and out of beds, baths, wheelchairs, or automobiles
• Care for patients by changing bed linens, doing laundry, cleaning the home, or assisting with personal care
• Maintain records of patient care, condition, progress, or problems to report and discuss observations with the supervisor
• Build relationships with patients by conversing or reading aloud to help keep them mentally healthy and alert
• Engage patients in exercises or other activities

Qualifications

• CNA Certification or PCA
• High School Diploma or GED
• 3-5 years of prior experience
• Driver’s license required
• CPR certification required
• TB test within last 1 year or chest x ray within last 5 year
