The IDD Care Coordinator develops and monitors individual treatment and support plans while facilitating the person-centered planning process. They also provide counseling, education, and guidance to empower individuals and train caregivers to meet specific needs.
Candidates must possess a Bachelor’s degree in a human services field as defined by Medicaid Provider Manual Guidelines. A valid Michigan driver's license is also required for this community-based role.
