Care Coordinators develop and implement individualized, cross-system care plans for youth with mental health diagnoses and severe behavioral disruptions. They conduct CANS assessments and collaborate with families and community supports to achieve long-term goals.
Candidates must hold a bachelor's degree in social work, psychology, or a related field and be eligible for registration as an Agency Affiliated Counselor. A valid driver's license and vehicle liability insurance are required, along with the ability to pass a background check and drug screen.
