The Care Facilitator manages patient care, collaborates with medical staff, and advocates for patients during acute episodes. They also monitor resource utilization and ensure safe, timely discharge planning.
Candidates must be a graduate of an approved nursing school with a current West Virginia Registered Nursing license and at least five years of hospital nursing experience. BLS certification is required, while a BSN or Master's degree and care management certifications are preferred.
Graduate of an approved school of nursing; holds and maintains current Registered Nursing License for the state of West Virginia.
BSN, Master Degree preferred, not required.
