Provide direct patient care and implement care plans under the supervision of a registered nurse. Responsible for patient education, clinical documentation in electronic records, and coordinating care with physicians and team members.
Must be a graduate of an accredited LPN or LVN program with a current state license and valid CPR certification. A valid driver's license with reliable transportation is required, and three years of nursing experience is preferred.

Graduate of an accredited Licensed Practical Nurse or Licensed Vocational Nurse program. Three (3) years nursing experience, preferred. Community, home health or medical/surgical experience is preferred. Currently licensed as a licensed practical nurse or licensed vocational nurse in the state. Possess and maintain valid CPR certification. Complies with accepted professional standards and practice. Demonstrates good verbal and written communication and organizational skills. Licensed driver with reliable transportation.
