The Community DSP provides habilitation services in community-based settings to support skill development, independence, and meaningful community integration for individuals with disabilities. They are responsible for organizing services that promote health, safety, and personal choice while accurately documenting patient information and participating in interdisciplinary team meetings.
Candidates must have a high school diploma or equivalent and a valid driver's license with the ability to satisfy agency driving criteria. The role requires functional proficiency in spoken and written English, the ability to lift 50 pounds, and the capacity to work in-person with extensive travel.

Under the supervision of the Manager or Senior Manager, the Community Direct Support Professional (DSP) provides habilitation services to individuals in community-based settings to support skill development, independence, and meaningful community integration. Services focus on assisting individuals with activities of daily living and building skills related to communication, socialization, transportation, problem-solving, money management, life safety, and other areas that promote independence and valued outcomes. Habilitation services are primarily community-based and typically center on recreational, volunteer, and social opportunities. The Community DSP is responsible for organizing and implementing services in a manner that promotes health, safety, and personal choice while adhering to all agency policies, procedures, mission, and values. This role requires the ability to speak, read, and write in English at a level sufficient to successfully complete required training, understand and follow support plans, communicate with care teams and providers, accurately document services and patient information, and to comply with health and safety standards.
