The Diabetic Program Coordinator oversees daily operations of the diabetes program and implements wellness initiatives to improve patient health outcomes. They manage reporting, coordinate patient care schedules, and facilitate educational classes in collaboration with medical staff.
Candidates must possess an Associate degree and a High School diploma, with 2 to 7 years of direct work experience. Proficiency in MS Office and strong communication skills are required, with bilingual English/Spanish skills preferred.
