The Community Health Worker facilitates connections between patients and community resources while providing culturally appropriate health education and support. They engage in outreach efforts, advocate for patient needs, and maintain thorough documentation in compliance with health center policies.
Candidates must possess a high school diploma or equivalent and be bilingual, with a preference for multilingual individuals. Additional requirements include a medical interpreting certificate, CPR/BLS certification, and the ability to use technology effectively.
The Community Health Worker is an individual who utilizes their experience and knowledge of the language and culture of the population he or she serves to facilitate bridging between patients and resources in the community. Community Health Workers provide health education and information and work with patients to effectively identify barriers to care and facilitate connections in the community to meet patients’ needs. The Community Health Worker should be comfortable with and have the ability to work effectively in a busy environment and can demonstrate flexibility to meet the changing needs of the patients and the Health Center. PRINCIPAL DUTIES AND RESPONSIBILITIES: Engage in outreach efforts as determined by the Health Center to “meet patients where they are” Provide culturally appropriate health education, information and, as needed, interpreter services for patients participating in BNHC Nutrition or other Health Groups, individual nutrition visits and Social Service visits. Communicate effectively with patients, community partners, and with an inter-disciplinary team of professionals at the health center. Provide direct services, including informal counseling and social support for patients Advocate for patients’ needs and support care coordination to meet those needs Complete all required documentation in a thorough and timely manner as required by Supervisor and per BNHC policy. Demonstrate HIPPA compliance and maintain patient confidentiality at all times. Attend all required meetings as determined by Supervisor QUALITY ASSURANCE Participate in weekly group supervision facilitated by a Supervisor. Participate in bi-weekly individual supervision with direct supervisor Participate in CHW training classes as determined by BNHC Complete all assigned myNetLearning Trainings QualificationsQUALIFICATIONS: High School Diploma or Equivalent. Bilingual Required. Multilingual Preferred Medical interpreting certificate for CVC, Portuguese, Spanish, Haitian Creole or French CPR/BLS Certified MINIMAL KNOWLEDGE BASE: Ability to read and write in English Ability to use technology effectively Understanding of the Brockton Community, available resources and needs of the population served WORKING CONDITIONS: BNHC is a busy community health center located in Brockton’s Downtown Business District. One late day per week may be required to accommodate patients attending evening appointments. Occasional travel as required per job responsibilities PHYSICAL REQUIREMENTS: Physical demands requiring lifting of small pieces of equipment and/or boxes not to exceed 20 pounds. Mostly sedentary and interoffice walking required. Driving and walking in the community as required. Visual acuity sufficient for frequent reading. Hearing acuity sufficient for holding conversations with or without audio devices.
