• You’ll Do: Our Case Manager make a difference in the lives of our patients. Our dedicated professionals work tirelessly to create an environment that fosters healing, growth, and long-term wellness. The Case Manager provides quality case management, psychotherapeutic services, and discharge planning to all patient and their families, serves as a member of interdisciplinary team supporting the organization's treatment program and philosophy and assure the deliverance of quality treatment to patients and their families. • Education &
• Experience: · Bachelors Degree in Social Work, Psychology or Social Services · Master’s Degree preferred · 3 years behavioral health experience preferred · Associate license such as ASW (Associate Clinical Social Worker), AMFT (Associate Marriage & Family Therapist), APCC (Associate Professional Clinical Counselor) preferred · Current BLS healthcare provider card required upon hire · Therapeutic Options™ training to be obtained prior to providing direct patient care without a preceptor
