This role is responsible for coordinating resident assessments, ensuring accurate documentation, and supporting quality care planning while maintaining regulatory compliance. The hourly rate for this job starts at $45/hr and increases based on years of experience. Responsibilities Coordinate and complete MDS assessments in accordance with CMS requirements and facility policy Ensure timely and accurate MDS submissions Collaborate with the interdisciplinary team to develop and update care plans; collect and validate supporting documentation from Nursing, Rehab, Social Services, Dietary, and Activities Monitor documentation to support PDPM and quality measures Participate in care plan meetings with residents and families Assist with survey readiness and audits Educate clinical staff on charting best practices that support accurate MDS coding and QMs. Review Quality Measures (QMs), 5‑Star metrics, and facility dashboards; help drive performance improvement initiatives. Requirements Active RN license in good standing MDS experience in skilled nursing or long-term care preferred Working knowledge of PDPM, QMs, Care Area Assessments, and CMS submission processes.
