The LPN will provide direct personal care to residents, including assistance with daily living activities and medical treatments under the supervision of the Resident Care Manager. They are responsible for monitoring patient conditions, maintaining accurate medical records, and communicating progress to doctors and families.
Candidates must hold a current certified LPN license and possess the physical ability to perform tasks such as lifting, bending, and operating medical equipment. The role requires familiarity with medical terminology, patient management software, and the ability to work effectively in a multidisciplinary team.

We are looking for a reliable LPN (Licensed Practical Nurse) to care for our patients in a professional and considerate manner. Provide personal care for residents under the supervision of the Resident Care Manager. Assist with basic resident care and daily living activities such as bathing, dressing, oral care, skin care, and meals, walking, and turning in bed as necessary which encourages each resident's ability to maintain the highest practical physical, mental, and psychosocial well-being. Maintain specific documentation as required.
Ensure patients have complete and appropriate medical care Observe patients’ medical records and report any relevant information to ensure timely treatment Monitor patients’ physical condition Administer treatments and medications and give basic care such as massaging, cleaning, bandaging etc. Consult with doctors or qualified nurses on an ongoing basis Evaluate patients’ progress and provide advice and counsel them accordingly Inform patients’ family or friends about their condition Maintain accurate patient records and assist in obtaining necessary medical documentation Skills Proven experience as LPN or similar role Ability to undertake a variety of diverse care tasks and work in a multidisciplinary team Familiarity with medical terminology and health care management systems Understanding of health care best practices Excellent knowledge of MS Office and patient management software (EPM) Excellent communication skills Compassionate and able to handle stress well Successful completion of LPN license program is a must
Current certified LPN. Physical and Mental Requirements: Ability to sit, stand, walk, talk, hear, use hands to finger, grasp, handle or feel, and perform repetitive motions of the hands and/or wrists; push, pull, reach, stoop, bend, kneel and climb. Ability to exert up to 50 pounds of force occasionally, and/or up to 20 pounds of force frequently or constantly to move objects. Moderate mental and visual attention required to perform machine operations and set up, read, and document resident information. Ability to operate a variety of medical equipment and instruments including, but not limited to hoyer lift, stretcher bed and wheelchair. Environmental conditions: Work is performed in a typical medical environment. Duties include handling garbage or soiled linen. There is substantial exposure to blood, body fluids and/or pathogens, infectious diseases, unpleasant odors and hazardous materials that require following extensive safety precautions and include the use of protective equipment.
