Inpatient Care Manager (RN)- Per-Diem, Days, 8 AM - 5 PM, Atlantic Health Chilton Medical Center

The Inpatient Care Manager is responsible for providing care coordination and services for selected patient populations across the continuum of illness. This role ensures effective utilization and monitoring of healthcare resources while managing interdisciplinary collaboration to achieve optimal clinical and resource outcomes. The Inpatient Care Manager works closely with the Supervisor or Manager of Case Management to assess, plan, implement, coordinate, monitor, and evaluate services and outcomes to maximize patient health and ensure safe, high-quality care.

Principal Accountabilities

Manage, coordinate, and facilitate patient care through comprehensive patient and family assessments and multidisciplinary collaboration. Perform daily chart reviews, utilization reviews, and coordinate discharge planning activities, ensuring thorough and accurate clinical documentation in the electronic medical record. Lead and participate in interdisciplinary patient care rounds, identifying and resolving quality of care issues, barriers to care progression, and discharge delays. Formulate, coordinate, and implement individualized plans of care and discharge plans to ensure safe, timely, and effective transitions. Collaborate with the interdisciplinary healthcare team to manage and support patient care plans. Perform variance tracking and outcome analysis to assess the effectiveness of clinical pathways and identify opportunities for improvement. Serve as a representative of the department and participate in hospital and community outreach activities as appropriate. Perform other related duties as assigned.
