The therapist will provide intensive case management and therapeutic services to members in a permanent supportive housing program. Responsibilities include conducting individual and group therapy, developing treatment plans, and coordinating care with multidisciplinary teams.
Candidates must hold a Master's degree in social work, counseling, or a related field and possess at least two years of direct clinical experience. Licensure in Tennessee or progress toward licensure is preferred, along with a valid driver's license.

This position is an advanced practitioner of therapeutic services in a Permanent Supportive Housing program that offers both Housing First and Sober Living options. The Permanent Supportive Housing Therapist will serve Park Center members on a recovery-oriented, housing-focused intensive case management (ICM) team.
A. Engage members in their homes and in office locations utilizing a trauma lens and cultural humility to quickly establish trust and build rapport. B. Partner with members to help them achieve goals around improving their mental health and substance use C. Provide individual therapy sessions for members with mental health challenges, substance use disorders, and physical health issues. Use evidence-based modalities such as Cognitive Behavioral Therapy (CBT), Dialectical Behavior Therapy (DBT), Motivational Interviewing (MI), and harm reduction strategies. D. Facilitate group therapy sessions focusing on topics such as trauma recovery, mental health management, addiction recovery, and coping with physical health issues. E. Support PSH members by offering recovery focused services that address wellness of both mind and body. F. Educate members about the connection between mental health, substance use, and physical health, providing tools to enhance overall wellness. G. Empower members by teaching coping strategies, life skills, and relapse prevention skills. H. Collaborate with members to develop personalized treatment plans that address mental health, substance use, and physical health needs. I. Provide crisis intervention for members in acute distress, ensuring safety and immediate support. J. Address mental health and substance use crises with sensitivity and a trauma-informed approach, coordinating with medical and social services teams as necessary. K. Assist PSH members with developing interpersonal skills to connect with members of the community and expand their natural support system. L. Complete, maintain, and review documentation for agency and funding requirements within all designated timeframes. M. Lead recovery focused groups in PSH housing for members to attend. N. Develop and provide resources to members based around recovery services. O. Assist PSH members with developing interpersonal skills to connect with their housemates and neighbors, and to expand their natural support system P. Create, maintain, and review documentation for agency and funding requirements within all designated deadlines.
A. Communicate with internal and external partners to coordinate care and advocate for member needs. B. Communicate regularly with the Permanent Supportive Housing team to meet member needs. C. Build rapport and connections with community service providers to provide wrap around support and care to members in recovery. D. Develop effective working relationships with human services organizations.
A. Attend all staff and supervision meetings. B. Assist with program events and outings. C. Maintain schedule flexibility in the event of after-hours responsibilities. D. Maintain good attendance and inform supervisor in advance of time off. E. Complete Relias Learning courses by deadline. F. Follow all Personnel Policies and uphold Park Center’s Code of Ethics. G. Perform all other duties assigned.
