• Location: Sauk City/Verona/Middleton Greenfield Rehabilitation Agency – Where Passion Meets Purpose Greenfield Rehabilitation Agency is seeking a PRN Speech-Language Pathologist (SLP) to support therapy services at our highly respected senior living communities in Sauk City, Verona and/or Middleton. • You’ll Do: Evaluate and treat adult and geriatric residents with communication, cognitive, and swallowing disorders Develop and implement individualized, patient-centered treatment plans Collaborate with interdisciplinary team members to enhance quality of life and function Accurately document services per company and regulatory standards

• Need: Current Wisconsin Speech-Language Pathology license and ASHA CCCs Excellent communication and documentation skills A passion for working with older adults in skilled nursing and senior living settings Professionalism, flexibility, and a team-oriented mindset
