The Registered Nurse will conduct initial home health assessments and develop comprehensive care plans for hospice patients. They are responsible for providing skilled nursing care and evaluating patient needs in their homes and nursing facilities.
Candidates must hold an active New Hampshire RN license and have at least one year of recent acute care experience. A valid driver's license, clean criminal record, and CPR certification are also required.
Baylor - Work 16 hours get paid for 24. BENEFITS ELIGIBLE! Saturday and Sunday only
About the Job
As a Registered Nurse in Homecare or Hospice, you’ll deliver skilled, compassionate nursing care directly in the patient’s home. You’ll complete comprehensive assessments—including OASIS—develop individualized care plans, and collaborate closely with physicians, families, and the interdisciplinary team to ensure high‑quality, goal‑directed, cost‑effective care. This role blends clinical expertise, autonomy, and relationship‑centered care to support patients in maintaining comfort, safety, and independence at home.
