The Case Manager coordinates client services, including court appearances, medical appointments, and therapy sessions, while maintaining accurate documentation and billing records. They act as a liaison between clients, families, and external agencies to ensure effective treatment plan implementation.
Candidates must hold a bachelor's degree in psychology, social work, or a related field and possess at least one year of experience in the mental health sector. Additionally, applicants must have or obtain a case manager certification within three months and maintain a valid driver's license.
Position: Outpatient case manager
Reports To: Case manager supervisor
The Case Manager will coordinate client services per the treatment plan for the adult clientele. Each case manager will be assigned specific clients at the time of intake and be required to program and monitor such things as court appearances, doctor appointments, family therapy sessions, and discharge referrals.

Bachelor’s degree in psychology, social work, mental health or related field. One year’s experience working in the mental health field serving diverse population. Possess case manager certification or obtain certification within three months of employment. Maintain a active driver license.
Ability to apply common sense understanding to carry out instructions furnished in written or oral form.
