Hospice RN

The RN Case Manager provides direct patient care services and manages patient care plans through systematic assessment of physical, functional, and psychosocial status. They coordinate delivery of services by consulting with physicians, team members, and families while maintaining accurate documentation of visits and travel.

Candidates must be a Registered Nurse licensed in Nebraska or a compact state with at least three years of nursing experience. A valid driver's license, reliable transportation, and BLS certification are required, with hospice or home care experience preferred.
