The Health Navigator serves as a primary care manager, facilitating service introductions and maintaining ongoing contact with participants. They perform comprehensive barrier assessments, assist with public benefit enrollment, and provide health coaching while supporting clinical staff.
The role prefers two years of experience in non-profit or community clinic settings and a personal or family history of systems involvement. Candidates must possess strong organizational skills, effective communication abilities, and a valid California Driver License.
