The Physical Therapist evaluates, plans, directs, and administers physical therapy programs to restore function, prevent disability, and help patients reach maximum independence based on physician diagnosis. Responsibilities include implementing individualized treatment plans using skilled techniques and communicating patient progress to the team.
require a Bachelor’s Degree in Physical Therapy, knowledge of best practices, sound clinical reasoning, and the ability to travel to various home care locations. Candidates must also possess current Pennsylvania licensure and American Heart Association Basic Life Support certification upon hire.

Master’s Degree Physical Therapy or Doctorate Physical Therapy 1 year clinical experience for Home Care Physical Demands Lift and carry 40-50 lbs.
Push/pull patients on bed, stretcher (requires 29 lbs. push force), lateral transfers up to 50 lbs. of the patient's weight. Frequent to continuous standing/walking. Patient transporters can walk 8-10 miles per shift. *Patient care providers may be required to perform activities specific to their role including kneeling, bending, squatting and performing CPR.
