Join our team as a Weekend Warrior RN, supporting patient care from Friday through Monday. This full-time position offers flexibility in scheduling.
For example, a typical schedule may start around 2:30
PM on
Friday and continue through
Monday morning (around 10:00 AM), but we are open to adjusting hours to fit the right candidate while maintaining weekend coverage. Schedule, Hours & Territory

Document assessments, interventions, and care plans accurately and in a timely manner in the EMR HCHB (Homecare Homebase)
Participate in interdisciplinary team meetings and care planning

Active Registered Nurse (RN) license
Hospice or home health experience preferred
Strong clinical assessment and care coordination skills
Ability to work independently in the field
Strong communication and documentation skills
Experience using HCHB (Homecare Homebase) preferred Field Role Requirements
Active driver’s license
Ability to pass a criminal background check
Ability to pass a Motor Vehicle Record (MVR) check
Current TB testing in accordance with agency policy
Current CPR certification as required for the role
