Senior Case Manager

• Seeking responsible individual working collaboratively with individuals who have a Mental Illness to support their wellness, recovery and independence within our Supported Housing Program based in Brooklyn, Manhattan and the Five Towns.
• Responsibilities include: regular apartment visits, weekly phone calls, medication and medical monitoring, contacting client’s mental health and medical health teams on a monthly basis.
• In addition, this position entails documentation of rental payments, fire drill evacuations and apartment inspections.
• Position requires a Bachelors degree in the Human Services field with strong background working with individuals who have a Mental Illness, Masters preferred.

Salary: Bachelors Level $65,000

Masters Level $70,000
