The Community Health Worker acts as a liaison between the healthcare system and the community to improve access to care and address social determinants of health. Responsibilities include conducting outreach, facilitating care navigation, and building partnerships with local organizations to support patient outcomes.
Candidates must have a high school diploma or GED and a valid driver's license with reliable transportation. Preferred qualifications include a college education in Social Work, Nursing, or Public Health, and a Community Health Worker certification.
