The Registered Nurse administers skilled nursing care to patients in their homes, including performing evaluations, initiating care plans, and documenting progress. They also coordinate with physicians, supervise other nursing personnel, and provide patient and family education.
Candidates must be a graduate of an accredited School of Nursing and hold a valid Florida RN license. A minimum of one year of experience in home health or a hospital setting is preferred, and a reliable vehicle with insurance is required.

A Registered Nurse administers skilled nursing care to patients on an intermittent basis in their place of residence. This is performed in accordance with physician orders and plan of care under the direction and supervision of the Director of Clinical Services/Clinical Manager.
Elite Home Health is Medicare certified and is licensed to provide comprehensive home care services in Broward, Palm Beach including North Palm Beach and Martin counties. We are ACHC Accredited and the agency is owned and operated by Registered Nurses, all with advanced skills and extensive experience in home health care.

Responsibilities: Of position:
· Makes the initial evaluation visit and regularly reevaluates the patient’s nursing needs.
· Initiates the plan of care and necessary revisions.
· Initiates appropriate preventive and rehabilitative nursing procedures.
· Prepares clinical and progress notes for each patient visit and summaries of care conferences on his/her patients in a timely manner as per Agency policy.
· Informs personnel of changes in the condition and needs of the patient.
· Teaches & counsels the patient and family/significant others in meeting nursing and related needs.
· Participates in and presents in-service programs.
· Understands and adheres to established Agency policies and procedures.
· Processes orders and notifies physician of patient needs and changes in condition. Completes certification/recertification orders and discharge summaries.
· Determines the amount and type of nursing needed by each individual patient.
· Refers to Physical Therapist, Speech Language Pathologist, Occupational Therapist and Medical Social Worker those patients requiring their specialized skills.
· Supervises and teaches other nursing personnel.
· Attends patient care conferences on patients assigned to his/her care.
· Gives total patient care as needed.
· Takes on-call duty weekends and holidays, as assigned.
· Completes and submits OASIS assessments, reassessments, transfers, resumptions of care, discharges and significant change in condition in accordance with CMC Guidelines.
· Evaluate the effectiveness of nursing service to the patient and family on an ongoing basis.
· Perform admission, transfer, re-certification, resumption of care and discharge OASIS for the home care patient.
· Must have knowledge of Medicare guidelines.
· Participate in the patient’s discharge planning process.
· Participate in the development and periodic revision of the physician's Plan of Treatment and processes change orders as needed.
· Performs supervisory visits as needed.
· May be requested by Director of Nursing to fill in for the other nurses.
· Must be a graduate from an accredited School of Nursing.
· Must be licensed in the State of Florida as a RN
· Minimum one year of experience in home health or hospital setting preferred, but not required. We will train.
· Travel is required; must have a reliable car w/current insurance coverage and a valid Drivers License

· Must have excellent written and verbal communication skills.
· Registered Nurse: 1 year preferred
License/Certification: · RN (Required)
· BLS certification issued by American Heart Association.
