- Summary: Vanderbilt Health Home Care Services (VHCS) Private Duty is a specialized program dedicated to delivering continuous, high-quality, patient-centered care in the home setting. As an extension of Vanderbilt University Medical Center, the Private Duty division supports patients with complex, chronic, and medically fragile conditions, enabling them to safely remain in the comfort of their own homes while receiving advanced clinical care. The department provides individualized, one-on-one nursing and personal care services, ranging from intermittent support to 24-hour care coverage. Care plans are developed collaboratively with physicians, patients, and families to ensure alignment with each patient’s unique medical needs, functional abilities, and personal preferences. Services are delivered by a multidisciplinary team that may include registered nurses (RNs), licensed practical nurses (LPNs), home health aides, certified nursing assistants, and personal care attendants, all trained to uphold Vanderbilt’s standards of clinical excellence and compassionate care. VHCS Private Duty plays a critical role in the healthcare continuum by bridging the gap between hospital discharge and long-term home-based management, reducing hospital readmissions, and promoting improved health outcomes. The program also provides essential education, support, and respite for caregivers, empowering families with the knowledge and resources needed to manage complex care at home. With a strong commitment to quality, safety, and regulatory compliance, the department adheres to state, federal, and accreditation standards while continuously advancing best practices in home-based care delivery.
